FOR COUNSELING
Call 503-846-3020 and request counseling services. A counselor will call you to do a phone screening and schedule an intake appointment. You can also fill out our request for counseling form and e-mail it to Marci Nelson, CCVS Manager or fax it to 503-846-3040 (be sure to write "CONFIDENTIAL" on your fax facesheet. You can also mail the form to:
