School-Based Care Coordination
Service Description
Care Coordination services are available to all youth in the district and their families. Students may be referred due to truancy, behavioral issues, or symptoms of mental illness. An assigned Care Coordinator will meet with the student and their family to complete an assessment, and then work with the family to find appropriate resources in the community. Care Coordinators will meet families at school, in their homes or in the community, and will assist with accessing mental health services, medical services, and basic needs. They will ensure that funding is available through insurance or other means and work to successfully link students and families to community resources. In addition, Care Coordinators work collaboratively with other involved agencies such as Child Welfare, Juvenile Justice, and other treatment providers.
Eligibility
Services are available to all students in the district and their families. In some school districts, insurance may be billed for care coordination services. No one is turned away for lack of insurance or an inability to pay.
