Washington County is committed to providing employees a competitive benefits package. It is with this commitment in mind that we will be introducing new medical plan options next year. These plans are designed to support and engage employees in their individual health needs, while sustaining affordability into the future.

Medical Options for 2015
Starting next year, each of our health plans will include two benefit levels; a "basic" and a "healthy choices". The healthy choices benefit level will offer lower deductibles and copays than the basic benefit level.
Complete the following two wellness activities by October 31, 2014 and you've earned the healthy choices benefit level.
Biometric Screenings - Wellness Activity #1Providence Health Plan and Kaiser Health's primary care providers can generally provide all of the biometric health information required to complete your personal health assessment. If you have recently had these tests and measurements listed below, ask them to provide this information. Typically a phone call or email to your provider will get you your recent results, an appointment in their office or refer you to a laboratory in your network. Note: Results may take up to 7 days.
Measurement criteria will need to include blood pressure, height, weight, cholesterol levels (HDL, LDL, triglycerides) and glucose levels.
For those employees not able to schedule this screening with their health care provider, onsite biometric screenings will be provided during Health and Wellness Week, August 18-22 and will be on a first come first served basis. Details on scheduling an onsite exam will follow in late July.
Personal Health Assessment - Wellness Activity #2
Providence Health Plan’s “Wellness Central” has been chosen for all employees to utilize the same personal health assessment. Employees will have the same experience regardless if you are a Providence Member or Kaiser Member.
Providence and Kaiser Members will first need to create a personal and confidential myProvidence account. Go to www.myProvidence.org and register. Once you have logged into your account, choose Personal Health Assessment on the left side navigation bar.

Privacy Reminder: Washington County is committed to respecting and protecting our employees' privacy. The County will only receive notification of your completion of the personal health assessment and will never receive your individual biometric screening or confidential online health assessment results.
