Mailing Address Changes

Mailing address changes can be requested in two different ways:

1.  Email your request to:

Please include the following information with your request:
    • Property account number
    • New Mailing address
    • Printed name of requester
    • Documentation if requesting on behalf of a trust, business, POA, etc.



2.  Please print and sign this document.  Mail it or fax it to the address/fax number listed on the formAddress Change Form (Adobe Acrobat; 449 KB, PDF)
