Choice is a program operated by Washington County to assist individuals in the highest levels of care transition back into their community. Primarily supporting individuals who have been committed or have been admitted to the state hospital, Choice care coordinators work with treatment providers and community services to support individuals to live as independently as possible. Service plans are driven by the client with care coordination provided by masters-level clinicians who are experts on the community system of care.
Washington County currently provides Choice care coordination services for individuals with behavioral health conditions who are OHP members assigned to Health Share of Oregon and Trillium Health Plans. Information about local OHP plans can be found on our OHP page.
