Housing Case Managers
The Housing Case Manager (HCMS) program funds approximately 15 community-based organizations and 40 case managers to work directly and continuously with a dedicated caseload of individuals who have experienced prolonged homelessness.
Housing case managers are trained in housing navigation, trauma informed cared, culturally responsive care, housing first practices and more. Working with 20 participants each, case managers build relationships to begin addressing each person’s barriers to housing, they help to navigate the complex housing placement process, and continue to support individuals with services and connections that support housing stability.
All referrals are coordinated through partnering HCMS service providers listed below, and Community Connect.
