Open Enrollment is held once annually, to allow for additions/deletions to current health plan elections. Open Enrollment also provides for changes between medical/dental plans.

Outside of the annual Open Enrollment window, you can only make changes to your medical, dental, vision, flexible spending accounts (FSAs) coverage if you experience a mid-year event that qualifies under the terms of the plan contracts and the IRS rules. All required enrollment forms must be received by Human Resources within 30 days of the qualifying event.
You can request changes to your voluntary life insurance amounts at any time. However, requests for increased life insurance coverage are subject to underwriting and may or may not be approved.
Termination of Coverage
Coverage under the Medical, Dental, Vision, EAP, FSA and Life Insurance Plans terminates at the end of the month in which you leave service. Under certain circumstances, coverage may also be terminated during an unpaid leave.
